CareSource 24-Hour Nurse Help Line: 1-866-234-0637

For non-emergency health situations, CareSource Members can call our Nurse Help Line 24-hours a day, seven days a week, including holidays. The call is FREE and confidential. Speak directly with a registered nurse about your symptoms or health questions. The nurse will quickly assess your situation and help you choose the most appropriate action. A fax will then be sent to your Primary Care Provider to help him or her coordinate better care for you.

We understand that health questions can pop up at any time. Call at your convenience to get the information you need when you need it. We're always here when you need health advice.

Disease Management Program

Our disease management program offers help for our members with the following chronic conditions:

Asthma, depression, congestive heart disease, hypertension (high blood pressure), diabetes and Chronic Obstructive Pulmonary Disease (COPD)

- Do you have questions about these chronic conditions?
- How do you obtain more information about these chronic conditions?

Case Management Program

Case management is a program for CareSource members who have one or more health care needs or disabilities. If you can answer yes to any of the following questions, the case management program may help you!

- Do you have many medical problems?
- Do you need help managing your health care?
- Do you have more than one health care provider?
- Do you have needs that might impact your ability to self manage your chronic condition (transportation, meals etc.)?

Who Will I Talk To?

When you call, you can work with a case manager. Case managers are registered nurses or licensed practical nurses . They can talk with you about your health care needs. They can also work with your providers to help you get the care you need.

Wellness Program

Our Wellness program offers the following to our members:

- Free membership at your local contracted  health club, currently the YMCA.

Walk’n’Rollers
– Free membership in our “walking and rolling group”
