The Plan contracts with over 200 Primary Care Providers (PCPs) in Josephine and Jackson County. Each CareSource (except our Diamond PPO) member needs to select a PCP from this group of providers. |
The Plan contracts with over 300 medical specialists from Mid Rogue Independent Physician Association in Josephine county and PrimeCare IPA specialists and sub specialists in Jackson county. In most instances, you will need a referral from your PCP to see a specialist (except for our Diamond PPO Plan). |
The Plan contracts with two hospital systems: Ashland Community Hospital in Ashland, Asante, which has facilities in Grants Pass and Medford; and Providence, with facilities in Medford and Portland. |
Allied health professionals |
The Plan contracts with a number of Allied health professionals to provide skilled nursing care, rehabilitation services, diagnostic services, and complementary medicine (chiropractors, acupuncturists and massage therapists). |
Our Service Area encompasses all of Josephine County and Jackson County as well as Glendale and Azalea in Douglas County. |
