Mid Rogue Health Plan is proud to present the family of CareSource Medicare Advantage Plans. CareSource provides coverage that goes above and beyond original Medicare, and because of the extended benefits offered by CareSource, you enjoy superior care in many areas.

We offer plans that include the Part D prescription drug option. Since CareSource is the only locally owned Medicare Advantage plan, we provide local control and response to the health care needs of our community.

Local doctors are involved in your health care on many levels, and we are proud to be affiliated with the highest quality physicians, hospitals and providers to assure the best health care available. Our providers include: Mid Rogue IPA (Josephine County) and PrimeCare (Jackson County) for primary and specialty care; and all local hospitals including Providence Medical Centers in Portland and Sutter Coast Hospital in Crescent City.

 

CareSource is a Health Plan with a Medicare contract. This contract is renewed annually and the availability of coverage beyond the end of the current contract year is not guaranteed. In case CareSource reduces its service area we will let you know 60 days in advance about your options in finding a new health plan or how you can go back to Original Medicare. You must continue to pay your Medicare Part B premium. You must use plan providers except in emergent, urgent care situations or out-of-area renal dialysis. If you obtain routine care from out-of-network providers neither Medicare nor CareSource will be responsible for the costs. (For  out-of-network coverage received under our Diamond (PPO) and our Platinum (HMO/POS) plans please contact our member services dept.)
